Appendicitis is the most common surgical emergency in childhood and the technologic advances of modern medicine have affected the diagnosis and treatment of appendicitis. This study is to evaluate the differences in diagnosis and treatment of appendicitis between present and 10 year ago. The authors retrospectively reviewed the medical records of patients who underwent appendectomy under the diagnosis of the acute appendicitis from July 1993 to June 1995 (Group A, n = 78) and from July 200 to June 2005 (Group B, n = 105). There are no differences between group A and B in mean age (8.5 ±3.6 vs. 9.3 ±3.1 year), duration of symptoms (3.0 ±3.2 vs. 2.6 ±3.8 days), and postoperative hospital stay(6.6 ±4.8 vs. 5.8 ±3.6 days). Preoperative abdominal ultrasonogram and/or computed tomogram was performed in 7 patients (9.0 %) of group A and in 51 patients (58.5 %) of group B. Thirty-six patients (34.3 %) of group B underwent laparoscopic appendectomy, but none in group A. Incidence of a histologically normal appendix decreased from 15.8 % in group A to 4.8 % in group B (p =0.018). This study suggests that utilization of abdominal ultrasonogram or computed tomogram in preoperative evaluation become more popular and surgical treatment of acute appendicitis become more minimally invasive. The rate of negative appendectomy was also reduced compared with 10 year ago.
The pyloric length, diameter and muscle thickness were measured by ultrasonograms of 15 infants with infantile hypertrophic pyloric stenosis (IHPS) and were compared with the infants who came in for the routine vaccination randomly. This study analyzed the changes in pylorus for those who received pyloromyotomy using the ultrasonogram at 4 weeks and 8 weeks postoperatively, such as the time needed to become normalized, and compare with the size of the hypertrophic pylorus before the pyloromyotomy. According to Carver5, the pyloric muscle volume (PMV) and pyloric muscle index (PMI) were calculated in each case. The pyloric muscle volume, PMI and the thickness of pyloric muscle proved to be a more reliable guide to diagnose IHPS than length and diameter of pylorus. The pyloric muscle after pyloromyotomy was measured by ultrasound at 4 weeks and 8 weeks postoperatively. The pyloric muscle length, diameter, thickness and pyloric muscle volume were not normalized at 4 and 8 weeks postoperatively. However, pyloric muscle index was normalized at 4 weeks postoperatively which was probably due to rapid weight gain after pyloromyotomy.
Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRI) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a 3.8 × 3.0 cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This findLl1g was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.
